Provider Resources

Health care providers play a critical role in improving maternal mental health by screening pregnant and postpartum women for perinatal mood and anxiety disorders (PMADs) and referring them to effective treatment. Below are patient questionnaires, as well as a reference sheet to help you screen and refer for PMADs.

If you would like to learn how to screen and refer women for PMADs, please see our free online and in-person trainings for health care providers.

Please note that the questionnaires below are meant to be completed by a patient to help facilitate communication with a care team in a clinical home environment. They are not diagnostic instruments and are to be used solely within the context of medical treatment with a care provider.
