INTAKE AND PATIENT CARE COORDINATOR

Part Time

Job Responsibilities:

  • Conducts intake interviews as well as needs assessments[CB1] 
  • Data enters all patient related information
  • Documents all interactions, including successful and unsuccessful outreach attempts
  • Answers and screens telephone calls from individuals inquiring about services
  • Compiles information regarding patient’s demographics, basic symptom and treatment history, and insurance benefit information to enter into electronic database accurately and in a timely fashion[CB2] 
  • Provides explanation of intake/insurance/referral process to clients
  • Completes financial requests in a timely fashion and presents/explores private pay options or financial assistance/sliding scale as required
  • Verifies insurance; forwards information to billing coordinator for follow-up[CB3] 
  • Assists in orientation and training of new clerical staff
  • Meets departmental metrics including % of calls to intakes, % of intakes to appointments
  • Generates computer reports that assist in the completion of department’s weekly statistics
  • Actively engages both potential and established clients in a highly responsive manner
  • Provides information about all of Seleni Institute’s programming to patients and families
  • Maintains follow-up contact including outreach calls or emails to patients and professionals

 

Education, Competencies, and Credentials:

  • BA/BS or equivalent in Human Services, Social Work, Psychology or related field required
  • 1st or 2nd Year LMSW students are open to apply
  • Demonstrated customer service skills required

  • Excellent verbal and written communication skills
  • Outstanding organizational and time management skills
  • High degree of professionalism and ability to maintain confidentiality
  • Proficiency with Mac, and Microsoft Office Products and clinical database systems
  • Ability to work well in a team setting
  • Demonstrated cultural competency and sensitivity to individualized patient needs
  • Takes ownership of work product
  • Completes work assignments thoroughly, accurately, and within established time frames
  • Work with the Clinical Director and the Clinical team
  • Ability to interact with clients in a friendly, helpful, and courteous manner
  • Ability to resolve patient complaints
  • Ability to assist patients in identifying and accessing community resources if needed
  • The ability the be flexable since this is a part-time position
